How to apply for or collect birth certificate for new born baby in Pune

Hello friends,

I was looking for information on how to apply or collect birth certificate of my daughter in Pune.

I explored on internet and found that the online system to apply and collect birth certificate was unfortunately shut down. And also did not get any information around the same.

So I have decided to visit the nearest ward office and collect information on how to apply for certificate. When I was reached at ward office (see below for more information on ward office), they said me to fill up the form. I am attaching form with this post below.

Documents you need to carry

  • Identify proof of mother or father. It can be PAN card, Aadhar Card, Driving Licence, Voter Id Card
  • Discharge card from hospital/Birth certificate from hospital.
  • Please carry original copy of proofs. It’s OK if you don’t have original copy of discharge card from hospital as many of us handed over to insurance companies to claim bill.

Where will you get the birth certificate in Pune

In Pune you can visit nearest Ward office of municipal corporation. Office have separate counter for birth/death certificate. You can fill up application and submit them with necessary documents listed above.

Below are the details you need to fill up in application form. Be ready with these information before. Please verify spelling twice of your child and parents name before submitting application.

  • Number of copies
  • Child’s name
  • Date of birth
  • Place of birth: Name and address of hospital
  • Mother’s full name
  • Father’s full name
  • Permanent address (this will be the same address you have given in hospital)

Fees

You will get only one copy for free of cost. You can demand more than one original copy to them. It costs 10 rupees per copy

Usually you will get birth certificate immediately on same day. Please verify spelling of name and address on computer.
